Clinical Pharmacokinetics of Vancomycin in Korean Neonates

Abstract
The clinical pharmacolinetics of vancomycin was evaluated in 19 Korean neonates with gestational age from 28 to 44 weeks. Initial vancomycin dosage was determined by the attending physician in according to the NEOFAX regimen based on postconceptional age (PCA). Vancomycin was administered intravencously over 60 minutes by a syringe pump. Blood sampling was done immediately before and 60 minutes after vancomycin administration. Vancomycin concentrations were measured by a fluorescence polarization immunoassay. Vancomycin pharmacokinetic parameters were calculated based on a one-compartment open model by Sawchuk and Zaske method. Kruskal-Wallis method was used for the comparison of mean pharmacokinetic parameters among three different PCA groups. Only twenty five percent of measured peak and trough levels were within therapeutic ranges. Vancomycin clearance showed linear correlation with PCA (r=0.567) but there was no relationship between volume of distribution and PCA (r=-0.346). Further prospective study is needed to develop now dosing guidelines and to confirm these pharmacokinetic data.
